Released a new module for PhpNuke.
Has been tested on phpNuke 6.5 to 7.1.
Ban Form was built for our gaming site @ www.teamstfu.com. We where getting a lot of forum crapping from players that have been banned from our site or gaming servers, this was meant to simply give them a form to fill out and mail to the admins without things getting drawn out for everyone to read.
Features:
It requires the person to fill in all fields before it will let them send it.
It retains all the information and adds it to the e-mail sent to the admins.
It obtains the persons current IP. (If it is a person that was banned from your site, it gives you the ability to keep them banned from your site. The main purpose was to beable to ban the trouble makers that go to the extreme to get back to your site and post more garbage.)
You can have it mail to one or more e-mail addresses.
